It would be helpful to know what the head circumference measurements were at birth and at previous visits to judge at what rate the head is growing. Nevertheless, a head circumference of 495 mm is definitely over the 95th percentile value of about 470 mm. Megacephaly just means a large head. Ventriculomegaly means that the ventricles, fluid filled spaces in the brain, are enlarged. The most likely cause of this is hydrocephalus caused by a ventricular obstruction. I will give you my opinion based on this assumption. I surmise that your child’s physician has already recommended consultation with a neurosurgeon concerning the placement of a shunt in the brain to drain off the fluid. While not perfect, a shunt is far, far preferable to the option of waiting or doing nothing. If nothing is done the head will continue to grow from the increased spinal fluid pressure, and the atrophic changes will increase, leading to severe retardation.
