Does anyone know a way to get a good upper-body workout with a torn rotator-cuff?

First, you have to rehabilitate your rotator cuff system, not just dissolve the scar tissue, though there are forms of medical intervention outside of surgery, but that portion should be considered friendly advice, because I’m not a doctor. Much of your problem with mass (or perceived mass) may actually be lack of sufficient muscle pump. You know that us weight lifters have very good intramuscular circulation. On that note, I’d recommend Arginine alpha ketoglutarate to help increase nitric oxide production and improve circulation. See this answer to one of my questions.
