I work at the Jobcentre 🙁 😛 Phone your local Benefit Delivery Centre. They’re the people who actually process the claims and work out who gets what. The Jobcentre should just send the form over to them. The ‘Warm Phones’ at your local JCP should have a direct line to the BDC. Tell them you believe you’re entitled to JSA and filled out a form that you don’t believe was needed. Then tell them that form’s gone missing and ask if you need to fill it in again or if the claim can be made without it.
