The I.D. card assists provider in identifying Superior Vision members and the plans under which they are covered. It is recommended that employees show the I.D. card to the provider; however, they can receive services without the I.D. card. Employees should simply identify themselves as Superior Vision Plan members with proper personal identification, I.D. number, and employer’s name.
