I don’t think you have the “right” to a midwife only to medical care. The Health Board is not going to employ midwifes to cover out of hours just in case a baby decides to arrive on a Sunday or during the night if the birth rate on the island does not warrant it. To cover 24hours a day, 7 days a week would require a minimum of 5 midwives to cover, day shift/night shift/days off/holidays/sickness.
