Do we still use the Service Application form to open a “cooperative” case after a CPS assessment when a child is determined to be unsafe?

Yes, for these cases we do use the 304, as that is the signed document that authorizes us to provide services to the family. We would have an In-Home Safety Plan, and the 333c case plan with those families as well.
