State agencies accepting peer counseling funds for the first time are required to submit an implementation plan demonstrating that they intend to implement all of the components of the Loving Support Model. Peer counseling implementation plans, including a line item budget with written narrative, are to be submitted as an addendum to the State Plan. State agencies receiving breastfeeding peer counseling funding in subsequent grant periods are not required to submit a new implementation plan, but must agree to continue to build upon and expand activities outlined in their original implementation plans, and fulfill reporting requirements. If a State agency’s peer counseling program changes significantly from the original plan, the State should submit an addendum describing the change.
