Each year we are required to have a DHEC Immunization Certificate. If you have a personal shot record you must have your physician translate it to a SCDHEC form. It is the parent’s responsibility to make sure that the immunization record is up to date and that the child has received all the immunizations necessary to enter school. If your child did not require new immunizations, the form from the previous year will be transferred to your file.
