The Enduring Memories™ Montage Order Form instructs customers to write their Matthews Customer Number and photo reference name or number on the back of each photograph. Photos are to be placed between two pieces of cardboard for mailing. They should never be stapled or paper clipped to the form. Photos can be submitted via mail, email or on a CD or zip disk.
