Response: No. The Infant Feeding Plan (IFP) should be filled out for Mostly Breastfeeding infants who receive less than ½ package, for example, if they only want 2 cans per month. Some Breastfeeding moms who want less than the full package will need the IFP part filled out on the Staff Communication Form. If at the certification, the mom receives ½ package or full formula packages, this can be indicated on the Participant Form and the Staff Communication Form does NOT need to be filled out. An exception could be if some benefits are to be issued from stock. Someone would need to determine how many cans to issue from stock and how many to issue on the card. During conversion to the new food packages, the Staff Communication Form should be filled out on all breastfeeding infants if they need formula.
