Once a sow has had a litter, you no longer need to worry about pelvic fusion. You do have the normal 5-20% risk of death that you have in any guinea pig pregnancy but no extra. She should be getting all the fresh 18% protein chow with vitamin C but without seeds, nuts, crunchy pits and colored garbage that she wants; all the fresh water she wants; all the good quality grass hay she wants and a moderate amount of greens. Reduce her stress by making sure she never runs out of food or water, limiting the amount you pick her up once she gets heavy and not traveling with her after about the 6th week unless it is an absolute emergency.
