Yes. Approved Individual members recieve 2 cards; one AmeriPlan Health (CDHC) ID Card and one Dental Program (DVPC) card. Approved household members will recieve 4 cards; two AmeriPlan Health (CDHC) ID Cards and two Dental Program (DVHC) cards.
