Do I risk relaying for spammers if I install MailSafe on a public IP?

No. The default MailSafe installation does not allow open relaying. This has been verified through the ordb service. If you need to let your users use MailSafe for outgoing mail, you can still do so, all they need to do is use authenticated SMTP (either through the password or login methods, which are available in most email clients).
