You do not need to complete a claim form when you submit a claim. All employees/retirees complete one claim form annually in September and all information needed is provided at that time. When you turn in a bill to Sage Technologies please verify that that your subscriber number and our group number is on the bill so Sage Technologies can quickly process it.
