Do I need to complete the Coordination of Benefits form?
Answer- Yes, anytime this form is requested please complete with as much information as possible. If there is another insurance for yourself or any dependents/spouse on the insurance, we need to notify the Health insurance companies to update the file. If there is not another health insurance, please mark the No Other Coverage area of the form. In the case of a divorce or custody issue, we need this information to notify the Health insurance companies and may require documentation to verify.
