Yes. The health form is a fundamental requirement and must be completed in its entirety. If you do not have a health record cleared by the College Occupational Health Nurse, you will not be allowed to enter the clinical portion of DENT 9017 (Dental Hygiene Refresher) and, therefore, will be unable to complete the course.
