You don’t HAVE to add cornstarch to pastry cream. But the cornstarch does two things for it. *Cornstarch thickens it *Cornstarch also gets in between the egg proteins so they don’t over coagulate (far less chance to curdle when they boil) and you won’t end up with scrambled eggs and milk.
