All members are sent ID cards with their welcome packet upon assignment to the plan that includes a member handbook, provider directory, health risk assessment and a postcard to request a PCP change. Members will have the opportunity to change PCPs by calling Customer Service at 1-877-347-9363, going to our website at www.fhp.org or mailing the post card back to us.
