Do funded programs automatically receive the full $15,000.00, or are there some who are funded with just DOH funds…..$5,000.00?
A. Each school that is selected will receive a $5,000 grant from the DOH, and up to $10,000 in matching funds. A separate budget must be prepared for the $5,000 base grant and the $10,000 match funds. Q. What do you mean by “interim physical assessments” as seen in Evaluation section of Narrative. A. The baseline and final school year assessment of the students must be made using the physical assessment documents listed as Attachment B1. The interim assessment was not intended to be as formal an assessment; rather, a midterm review of how the program may be progressing, identify any barriers and address both program and fiscal operations for any necessary improvements.
