United Concordia FEDVIP members are not required to bring their ID cards to the dentist. However, each member’s ID card displays his/her individual unique member identifier (UMI), which could be useful to the dentist to submit claims and/or retrieve eligibility information. If a member visits an Advantage Plus network dentist, it is not necessary for him/her to bring a claim form; United Concordia’s network dentists take care of all paperwork. However, employees may need to submit their own claims if they visit a non-network dentist. They can print a claim from the Forms & Publications page, and should bring it to the dentist’s office for their appointments.
