• Providers do not need to be contracted with MHP to provide services to MHP members. • Providers must be certified by the Minnesota Department of Human Services (DHS) as an ARMHS provider. • Providers must be certified in each county where they provide services. Providers must register with MHP by completing the Provider Information Form (PIF) and submitting their PIF, W-9, and DHS ARMHS Provider Certificate to MHP Contracting Department, 400 South Fourth Street, Suite 201, Minneapolis, MN 55415, or faxing to MHP Contracting, 612-904-4324.
