If the lead section of the form is not completed by the child’s health care provider, a nurse can administer the questionnaire and mark that section “yes or no”, refer for the blood test if indicated and note the date of the referral on the form. 3. There is no requirement to get the actual blood test results. If the lead section of the health examination is not completed by the health care provider or nurse, the form is incomplete and cannot be accepted for school enrollment.
