No. While there is not a pro-forma, a testing officer certificate of authorisation should include the following information: • name of rail transport operator • the full name of the authorised officer or position title of the authorised position • statement that the testing officer or position is authorised under clause 4 of the Rail Safety (Drug and Alcohol Testing) Regulation 2008 for the purpose of alcohol testing, drug testing or both. A person who is required to undergo testing may ask to see the certificate of authorisation. A certificate of authorisation may take the form of an identification card (similar to a driver’s licence, with or without a photo) for ease of reference. Where a specified position is approved as a testing officer, evidence that the testing officer currently holds the specified position is equivalent to a certificate of authorisation.
