If your eardrum is perforated then you actually have a big advantage, you will have no problems with the pressure changes that cause the pain (the pain arises from pressure difference between the outer and middle ears: with a hole the outer and middle ears become, effectively, one space). From your doctor’s description, I’m not sure you do have a perforation, but ‘just’ an external ear infection.
