Can the Voluntary Acknowledgment of Paternity Affidavit be rejected?
Affidavits without notarized signatures for both parents, or with corrections or white-out will be rejected. The birth record on file also includes information about whether the mother was married at any time during the pregnancy. Affidavits will be rejected if the birth record includes information that the mother was married.
