Can the Support Broker also be the Case Manager on the same plan of care?

No. The Support Broker can only provide Support Broker services and nothing else on an individual’s plan of care. If the provider or provider agency is currently providing case management services, or any services on the plan of care, they cannot be a Support Broker for that participant.
