No. The original MOLST forms have undergone an extensive review process with the NYSDOH in 2005. The forms revised in October 2005 are consistent with New York State Law and are approved for use by NYSDOH for all health care facilities in New York State. However, additional guidelines for starting/stopping treatment not addressed elsewhere on the form can be included in Section E under “Other Instructions,” for example, decisions about dialysis, implantable defibrillators, and the duration of time-limited trials.
