Can someone, other than the parent or legal guardian, bring a child in for an appointment?
Vital Smiles prefers to talk with each parent, but sometimes due to scheduling conflicts parents are not always able to accompany their child to the office. Vital Smiles utilizes a “Letter of Consent” for such circumstances. If a parent/guardian needs someone else to represent them during the appointment, then the signed Letter of Consent must be brought in at the time of the appointment. The parent’s representative will complete and sign the necessary office forms and give consent for any treatment completed.
