If you are experiencing difficulty having the physician provide information to Sedgwick CMS, please contact us with the physician’s fax number and we can fax a form to their office in an attempt to obtain the required objective medical information. If your employer has established procedures with Sedgwick CMS to call the physician upon initial filing, we will phone the physician for you.
