If you are experiencing difficulty having your physician provide disability information to Sedgwick CMS, please contact us with your physician’s fax number and we can fax a form to your physician’s office in an attempt to obtain the required objective medical information. If your employer has established procedures with Sedgwick CMS to call your physician upon initial filing, we will phone your physician for you.
