Can providers fax corrected claims for additional payment?
To correct minor errors and omissions such as changing dates of services, number of units, modifiers, etc., this information can be faxed. However, providers may wish to utilize the Reopening Adjustment Request Form. This way, the requests are tracked and handled in a timely manner. All reopening requests must be made within 12 months of the original claim remittance date. When faxing a reopening request, we ask that providers limit the number of patients (i.e. no more than 5) per form. However, in order to ensure the proper and accurate handling of your request, we suggest that you only include one patient per form.
