Can other members of my household use my ChaseHealthAdvance Revolving Account?
Yes, a ChaseHealthAdvance Account Holder may authorize members of their household to sign a Transaction Receipt and incur charges to the account holder’s account without the account holder present. The Account Holder must first request and fill outthe Authorized User Form listing those members of their household that they approve to use their account. ChaseHealthAdvance will then process a transaction for the authorized user, who must provide proper identification.
