Yes, you may have your doctor complete the Fax Prescription Order Form (PDF). It is necessary for you, as the member, to complete the member, patient and payment information prior to giving the form to your doctor for completion. It is important that you use the correct form since the incorrect form can result in delays or unfilled prescriptions. Additionally, if you have not registered with Prime Therapeutics, LLC, you will need to complete the Mail Order Form (PDF). For more information, download our Mail Order Pharmacy brochure (PDF). Note: In order for the prescription to be valid, it must be faxed from the doctor’s office.
