Can my candidates statement and reference be supplied on separate sheets of paper?

Yes. The reference can also be sent separately (signed on headed letter paper) from the application form, but must be received before the deadline. The candidate’s statement must be sent at the same time as the application form, and must be typed and no longer than one sheet of A4. Everything on the checklist (last page of the application form) must be received by the deadline for the application to be processed.
