Medicare providers are required to submit MSP claims and adjustment electronically using the American National Standard Institute (ANSI) ASC X12N 837 format, unless they meet the small provider exception or one of the other exceptions noted in CMS Pub. 100-04, Ch. 24, §90 . MSP claims or adjustments submitted after October 5, 2009, via direct data entry (DDE) (directly into the Fiscal Intermediary Standard System (FISS)) will be returned to provider (RTP) with reason code 31265 (claim is submitted or updated via DDE and the payer code is not equal to ‘Z’).
