Yes you can. This is called Proxy access. Proxy access allows you to view another person’s health information or communicate with that person’s health care provider. Proxy access is available for the following relationships: • Parent (must be 18 years of age or older ) to Child (up to age 18) • Legal Guardian to Patient (Ward). Legal Guardian must provide probate court documentation. • Adult to Adult Patient with an OSUMyChart Account (patient must have OSUMyChart account) No one under 18 years of age can be a proxy, request a proxy, or have their own OSUMyChart account. To request this convenient service, a Patient Request for OSUMyChart Proxy Access form can be completed at OSU Medical Center outpatient locations using OSUMyChart. The form must be completed in the office by the adult patient, parent, or legal guardian for ID verification purposes.
