Yes you can. This is called Proxy access. For pediatric patients this allows a parent (or guardian) to log into their personal MyChart account, and then connect to information regarding their family member. Complete a Proxy Consent Form at the time of the childs visit and you will receive the proxy information via U.S. Postal mail. Note: this is only for pediatric patient from the ages of birth through 11 years of age. At age 12 this privilege is automatically revoked. For adult patients this allows another person, of the patients choosing, to log into their personal MyChart account, and then connect to information regarding the patients information. The patient will need to complete a Proxy Consent Form at the time of their visit and the proxy information may be given to the patient or the proxy at the time of the visit. This privilege can be revoked by the patient at any time.
