Can I submit a signed field placement or employment verification form with the application?
Typically, NCTRC mails out the verification form to the respective field placement supervisor or employer and awaits return with signature, however, the signed verification form can be submitted with the application if the signature of either the field placement supervisor, or human resource designee is notarized.
