Can I obtain the cover page to facilitate institutional signatures before completing the on-line application?
You can print a cover page prior to completion of your application. You will need to begin the application process by completing the first two pages then skip forward to the Personal References and provide the three names of those writing a letter on your behalf. One letter must be from the Chairman of your Division or Department, and two from appropriate physicians and/or scientists who are familiar with your work. This will allow you to reach the end of the on-line process to print out the cover page and will still allow access back into your application at a later time to complete and upload the full application.
