You must apply for prescription drug benefits and be approved for the Illinois Cares Rx program. To apply, complete Form IL-1363 and, if you want Illinois Cares Rx for: • yourself, complete Section F on Form IL-1363, or • your spouse, complete Section G on Form IL-1363. If you decide after your Form IL-1363 has been approved that you would rather receive the $25 rebate instead of your prescription drug benefits, you can call toll-free 1-800-226-0768 to request a switch in your benefits under the Illinois Cares Rx program.
