No. Faxed documents will not be accepted. Hard copies of the two-page Signature Page form and the Institutional Nomination Form with original signatures, must be received by BWF no later than 4:00 p.m. Eastern Time on the application deadline. Using a courier service to send both forms is strongly recommended.
