You may convert your Intel basic and supplemental life insurance into an individual converted life policy without a medical examination if you apply in writing within the required time period (31-days from termination) and pay the required premium. The amount of life insurance you are entitled to convert will not be more than the amount of group life insurance being replaced. The minimum amount of your life insurance that can be converted is $500. You may elect to convert, without medical certification, all or any part of your employee supplemental AD&D insurance, except amounts less than $25,000 and amounts in excess of $250,000, if you apply in writing within the required time period, pay the required premium, and are not already carrying another converted policy for AD&D insurance that was converted from a previous employer’s plan. You are eligible to convert AD&D coverage if under age 70. Once converted, coverage ends at age 70.
