You are eligible to apply for a Kaiser Permanente for Individuals and Families plan if you live on the islands of Oahu, Maui, or Hawaii (except for ZIP codes 96718, 96772, and 96777). Kaiser Permanente for Individuals and Families plans are not available if you live on the island of Kauai.
