Yes, employees may waive medical coverage for themselves or their dependents if they are covered by DSHS. These employees should be cautioned that DSHS may not cover certain services that PEBB medical plans cover. To re-enroll in PEBB coverage, the employee must complete an Employee Enrollment/Change (722.9 KB) form (HCA 50-400) and provide proof of loss of the DSHS coverage within 60 days from the loss of coverage, or wait until the annual open enrollment period. Note: Employees may waive medical and dental on their dependents. They may waive only medical on themselves. If an employee waives medical coverage for himself/herself, medical coverage is automatically waived for all eligible dependents.
