a. NO. Only one (1) DOL provider number may be assigned for all individual providing services at that location. Therefore the practice should be enrolled as a Group with each practitioner being associated with the Groups Provider number. However, if you need to enroll under this tax ID/EIN/SSN an explanation must be provided and a paper enrollment submission will be required via fax for the appropriate OWCP program. Currently Provider Enrollment does not issue multiple provider numbers to members in a group unless they are in a different location.
