There are two possible solutions to this: 1. To make the incision just below the eyeliner, leaving it untouched. However, the scar, 1 or 2 mm below the ciliary rum, might form a “ridge” as a result of skin adhesions. 2. The best solution would be to go for transconjunctival blepharoplasty by CO2 laser (this cuts into the conjunctiva inside the lid and leaves no visible scarring) in conjunction with laser treatment to re-tighten the skin of the eyelid, thereby leaving the eyeliner untouched. In the case of upper blepharoplasty, the eyeliner is in no danger, for the incision is made in the palpebral fold. We should simply point out that if the eyeliner tail is long it could end up slightly raised.
