I give them cards (like book marks) with space for 3 targets on each side – maybe in thought bubbles or something. I write a target in the first one. Their parents initial it, they bring it back to school, and then the child has the card on its desk during all writing lessons. When they have done the target three times consistently I give them a target met sticker on the card by the target, I date it, and give them another. When they have done three targets (filled up one side of the card) the get to choose a prize (a nice pen, extra golden time etc). I find it really works and gets them to take control of their own learning. I take targets from level descriptors sometimes, but sometimes they may be as simple as correcting letter formations, or even writing on the line! It’s really easy to differentiate them, which is good. The same thing can be done for maths and reading.
