ISPs could choose to offer both filtered and unfiltered connections. However, while implementing the filter for all connections will be quite easy, it would get a lot more complex if they wished to offer both filtered and unfiltered internet connections. If the ISP wishes to offer this they will need to put in a significant investment to duplicate/upgrade their core internet routing infrastructure.
