If you really want, you can do two separate fellowships. Hospital medicine (or hospitalist) fellowship is now a separate fellowship beyond internal medicine residency. However, you do not have to complete this fellowship if you want to become a hospitalist. However, if you want to become a cardiologist you will have to complete a cardiology fellowship. You can be an internist or hospitalist who has a particular interest in cardiology, but to be board certified in cardiology and perform their procedures/tests you must complete a cardiology fellowship. In reality, you won’t want to or need to be board certified in both areas.
