Can a provider use the Panel’s Disclosure and Consent—Medical and Surgical Procedures form to obtain consent to perform a hysterectomy?
The Consent Statute provides that the form specifically established for hysterectomy (25 Texas Administrative Code Sec. 601.8) is to be used for that procedure, unless the hysterectomy must be performed in a life-threatening situation in which the physician determines obtaining informed consent is not reasonably possible.
